Diocesan Disciple Maker Index Survey
Hello St. Joan of Arc Staff/Parishioner!
Our Diocese of Phoenix is undertaking an initiative partnering with the Catholic Leadership Institute to offer a survey. Your participation in the Disciple Maker Index survey is warmly welcomed. This survey will provide you with the opportunity to reflect on your own spiritual growth, attitudes, beliefs, relationship, and satisfaction with the parish, participation and provide additional feedback on the efforts of the parish to help you grow. The survey should take you between 10-15 minutes to complete. Please do not skip questions unless instructed to do so. Please note that your responses are kept confidential.
Our parish of Saint Joan of Arc and Catholic Leadership Institute will not have access to individual survey responses. In February, we will have some data to present to the parish for our own growth and improvement on matters regarding the salvation of our souls in our particular parish family.

We will also have “paper” survey’s available for your use, following Mass in Santa Maria Hall the weekend of Nov 29/30 and Dec 13/14.
